One application per household, please. A physical U.S. residential address is required; P.O. boxes are not accepted. Please have accurate family information available. We may request supporting documents and a valid, unexpired U.S. state ID or driver's license later, if needed. Do not upload or email identity documents with this initial application. Further instructions will be provided if your application advances.

Families may create a wishlist by saving products from Utopia Depot. The wishlist can generate a shareable link and QR code that sponsors can scan. The wishlist is separate from this private application.

5. Important terms and acknowledgment

No guarantee of acceptance or assistance. An application is only a request for consideration. Submission, screening, or a follow-up message does not guarantee approval, sponsorship, any particular gifts, delivery, or other assistance. Participation is subject to eligibility review, verification of information, program capacity, and the availability and decisions of participating sponsors.

Eligibility and verification. Utopia Depot's Hope for the Holidays program may ask for reasonable supporting documentation of household circumstances, income, parental or legal-guardian status, residence, and a current, valid, unexpired U.S. government-issued identification card or driver's license. Any such documentation will be requested separately with further instructions. Please do not submit identity documents through this form.

Accuracy, household limits and program discretion. Submit only one application per household. Duplicate, incomplete, misleading, or unverifiable applications may be delayed, declined, or removed from consideration. Program administrators may determine eligibility and allocate available sponsorships in accordance with applicable law. There is no fee or purchase requirement to apply.

Your family's privacy matters. Your application details—including parents' and children's full legal names, residential address, phone number, email address, household income, children's birthdates, and personal statement—are confidential. We will not publish this information or give it to sponsors or other applicants. It is used only by authorized program administrators to review applications, verify eligibility where necessary, and communicate with applicants. Shopify and essential service providers may process application information to deliver or protect the service, and disclosure may occur where legally required. Any public wishlist contains only the display name you choose and the products you add, not your application details. Please review our Privacy Policy. Never include Social Security numbers, identity-card images, bank details, or unnecessary sensitive information in your story.
